REAP HEALTH COMMUNITY CARE - REHECOCA
REAP Health Community Care (REHECOCA) is an inclusive, charitable, not-for-profit Community-Based Organization established in December 2021 and fully registered under Wakiso District Local Government (Reg. No. WCBO/23/2059). The organization is committed to improving access to essential health services for women, youth, girls, young mothers, orphans and vulnerable children (OVCs), persons affected by HIV/AIDS, persons with disabilities (PWDs), widows, the elderly, and immobile persons, especially in underserved peri-urban and rural communities of Wakiso District. The organization operates RIHAB Medical Center as its flagship project, a community-oriented facility designed as a one-stop health hub providing outpatient, maternity, laboratory, imaging, and rehabilitation services to local populations.
Through the “Reaching the Unreached: Integrated Home-Based Care for Vulnerable Populations” project, REHECOCA uses the Home Based Care (HBC) approach as a community-centered approach to delivering health and supportive services to individuals and families in their own homes more particularly for managing chronic illnesses, disabilities, and conditions requiring long-term care, including but not limited to HIV/AIDS, tuberculosis, elderly care, and palliative services. Through the approach, we have been able to reach out to 200 + community members with health care services, food relief and other livelihood support, including training for Primary care givers in patient and elderly care in the last three years. Any support towards this cause will boost the organization’s capacity to extend this support to many underserved individuals that we are currently unable to reach out to.
Mission Statement: “To ensure quality wellbeing of the Ugandan society through improving and increasing access to grassroots community health through health education and promotion (lifestyle, hygiene, and nutrition), provision of reproductive, neonatal, child, and adolescent health (RMNCAH) services, and essential clinical care services including rehabilitation and palliative care services, HIV/AIDS inclusive.”

Why Support Us
Reaching the Hard-to-Reach
REHECOCA works directly in underserved peri-urban and rural communities of Wakiso District, reaching individuals who are often left out of mainstream health systems like the elderly immobile patients, persons with disabilities, women, widows, girls, orphans and other vulnerable children (OVCs), young mothers, and people affected by HIV/AIDS. Donor support ensures that health and other social economic services reach those who need them most, right in their homes.
Proven Community-Based Impact
Through its Home Based Care (HBC) approach, REHECOCA has already reached 1000+ community members over the past three years with: • Essential health care services • Food relief and livelihood support • Training for primary caregivers in patient and elderly care • HIV Counseling & Testing services • Sexual Reproductive Health & Rights services • Maternal & child Health services This demonstrates practical experience, trust within the community, and capacity to deliver results.
Integrated “Clinic-to-Community” Model
REHECOCA operates RIHAB Medical Center as its flagship project, in-patient, providing outpatient, maternity, laboratory, imaging, and rehabilitation services. This enables a strong referral and follow-up system where patients receive facility-based care and continued support at home, an efficient, cost-effective, and sustainable health delivery model.
Cost-Effective Use of Resources
With an estimated budget of USD 50,000 (UGX 180,000,000), REHECOCA plans to support 500+ people in one year, showing strong value for money and high impact per dollar invested.
Focus on the Most Pressing Health Needs
REHECOCA addresses chronic and long-term health conditions that place heavy burdens on families and communities, including: • HIV/AIDS and tuberculosis • Elderly and palliative care • Disability-related care • Maternal and child health needs • Non Communicable Diseases Donor support directly contributes to improved quality of life, dignity in care, and reduced preventable complications.
Capacity Building and Sustainability
• Beyond service delivery, through the home-Based Care Approach REHECOCA invests in: • Training community and primary caregivers • Strengthening household-level health management • Reducing dependency on emergency and hospital-based care This approach promotes long-term resilience and sustainability within communities.

REHECOCA is a fully registered Community-Based Organization under Wakiso District Local Government, ensuring: • Legal compliance • Community accountability • Transparent implementation of donor-supported activities
Opportunity to Scale Impact
Despite its achievements, REHECOCA currently cannot reach all those in need due to limited resources. Donor support will: • Expand home-based care coverage • Increase access to essential medicines and supplies for the vulnerable community members, reach many more vulnerable individuals currently unreached • Equip RIHAB medical center units like the Dental, Imaging, Theatre, Laboratory with the requisite modern equipment to provide quality health services to the community members. Partnering with and Supporting REAP Health Community Care means investing in dignity, access, and equity in health care & other social economic services for vulnerable populations. Donors partner with a community-rooted organization that delivers real impact at household level, strengthens local capacity, and ensures that no one is left behind.
